Kindergarten registration for the 2016-2017 school year will be April/Abril 18th – 22nd, 2016. Click here to see the information flier.
Here’s what you’ll need.
Please Pick-up The Following Forms At Your Child’s School during the week of April 18-22, Complete & Return To The School:
• Student Registration Form
• Certificate of Immunization Status Form
• Student Health Information
• Special Programs Screening Form
• McKinney-Vento Form
• PACKETS ARE NOT AVAILABLE UNTIL WEEK OF REGISTRATION
When you return these forms you will also need to bring in:
*Your child’s immunization record
*Your child’s birth certificate
*Your child’s social security number (optional)
*Proof of residence (for example: utility or rent bill - showing
your name & address)
*Emergency Contacts: 2 names, addresses, and phone numbers
