Wenatchee High School 9th Grade Mental Health Questionnaire
Information for Parents and Students
Wenatchee High School will offer a Mental Health Questionnaire to all 9th grade students this coming Tuesday, September 24th. The questionnaire encourages students to respond about their mental health and will be used to offer additional support to reduce mental health risks. The Questionnaire is voluntary. Students choosing to participate will be asked to complete the questionnaire in the auditorium at the end of one of their morning classes. Students taking the questionnaire can skip questions and stop taking the questionnaire at any time. Students can also remain in their assigned class, rather than report to the auditorium. If a student decides, while in auditorium, that they wish not to participate they can choose to leave an anonymous questionnaire in the box on their way to their next class.
Students who choose to share their name and important information about their well-being in the questionnaire may be contacted for further conversation and screening throughout the day and the coming weeks ahead. On Tuesday, we will have several community agencies on hand to assist with the additional screening and follow up needed to support all our students.

Q: Who will be asked to take the questionnaire? A: Students in Grade 9
Q: What questions are on the questionnaire?
A: Questions about interest, mood, appetite, concentration, energy level, self-harm, and suicide ideation. The Questionnaire is a well documented and commonly used mental health screening tool. Many of you might have been asked the same questions, if you have seen a doctor recently.
Q: How is student confidentiality protected, especially if they choose to identify themselves?
A: If a student chooses to identify themselves by providing their name on the questionnaire, the student may be discreetly contacted (as well as parents) with minimum attention for further conversation and screening to insure the safety and wellbeing of the student.
Q: How are the questionnaire results used?
A: Results will be used to understand our students and to provide them with the services they need for mental health and wellbeing. Results are also used for planning, evaluating, improving programs and services to our students and families.

Cuestionario de salud mental de noveno grado de la Preparatoria de Wenatchee

La Preparatoria de Wenatchee ofrecerá un cuestionario de salud mental a todos los estudiantes de noveno grado el próximo martes 24 de septiembre. El cuestionario alienta a los estudiantes a responder sobre su salud mental y se utilizará para ofrecer apoyo adicional para reducir los riesgos de salud mental. El cuestionario es voluntario. Los estudiantes que elijan participar deberán completar el cuestionario en el auditorio al final de una de sus clases de la mañana. Los estudiantes que toman el cuestionario pueden saltear preguntas y dejar de tomar el cuestionario en cualquier momento. Los estudiantes también pueden permanecer en su clase asignada, en lugar de presentarse en el auditorio. Si un estudiante decide, mientras está en el auditorio, que no desea participar, puede optar por dejar un cuestionario anónimo en la caja de camino a su próxima clase.
Los estudiantes que eligen compartir su nombre e información importante sobre su bienestar en el cuestionario pueden ser contactados para una mayor conversación y evaluación durante todo el día y las próximas semanas. El martes, tendremos varias agencias comunitarias disponibles para ayudar con la evaluación adicional y el seguimiento necesario para apoyar a todos nuestros estudiantes.

P: ¿A quién se le pedirá que responda el cuestionario? R: estudiantes en el grado 9
P: ¿Qué preguntas hay en el cuestionario?
R: Preguntas sobre interés, estado de ánimo, apetito, concentración, nivel de energía, autolesiones e ideas suicidas. El cuestionario es una herramienta de detección de salud mental bien documentada y de uso común. Muchos de ustedes podrían haber recibido las mismas preguntas, si han visto a un médico recientemente.
P: ¿Cómo se protege la confidencialidad del estudiante, especialmente si eligen identificarse?
R: Si un estudiante elige identificarse al proporcionar su nombre en el cuestionario, se puede contactar discretamente al estudiante (así como a los padres) con la mínima atención para una mayor conversación y evaluación para garantizar la seguridad y el bienestar del estudiante.
P: ¿Cómo se usan los resultados del cuestionario?
R: Los resultados se utilizarán para comprender a nuestros estudiantes y brindarles los servicios que necesitan para la salud mental y el bienestar. Los resultados también se utilizan para planificar, evaluar, mejorar los programas y servicios para nuestros estudiantes y familias.
