Nurses/Student Health Forms
A Medication Authorization Form is required anytime a student needs to take medication during the school day or during school-sponsored activities and events that occur outside of regular school hours, such as field trips, athletics, or overnight trips. Forms must be completed and signed by both the parent/guardian and the student’s healthcare provider before medication can be administered or carried at school. A new form is required at the start of each school year, as well as anytime there is a change in medication, dosage, or instructions. Please contact your school nurse with any questions.
An Emergency Medication Authorization Form is required anytime a student needs emergency medication at school, including epinephrine and inhalers. Forms must be completed and must be submitted prior to the student’s first day of attendance each school year in accordance with WAC 392-380-020. A new form is required at the start of each school year, as well as anytime there is a change in medication, dosage, or instructions. Please contact your school nurse with any questions.
